Sliding Scale Rate
A limited number of sliding scale spots are available on a first-come, first-served basis for patients experiencing financial hardship. The rate is $40–90 per one-hour treatment.
Injection services are not included in the sliding scale discount.
Topical liniments, ointments, and supplements are available at 10% off.
How to Apply
Email me with your full name to join the waitlist. When a spot opens, I'll reach out to collect your information and get you set up in the patient portal.
Documentation required. I will ask for one of the following forms:
Proof of unemployment
Medicaid card
Pay stubs (past 3 months)
Most recent tax return
Before your first appointment
You will need a credit card or HSA card on file to schedule. It won't be charged unless there's a no-call/no-show, in which case the full treatment rate applies.
A Superbill (itemized invoice with billing codes) is available upon request to submit to your insurer for potential reimbursement.
